|The following report is the Community Health Needs Assessment (CHNA) prepared on behalf of Mercy Medical Center - Centerville (Primary Service Area residents) for Appanoose County, IA. |
Creating healthy communities requires a high level of mutual understanding and collaboration with community individuals and partner groups. The development of this assessment brings together community health leaders and providers, along with local residents, to research, prioritize county health needs and to document community health delivery successes. This health assessment will serve as the foundation for community health improvement efforts for the next three years. The Patient Protection and Affordable Care Act (ACA) require non-profit hospitals to conduct a CHNA every three years and adopt an implementation strategy to meet the needs identified by the CHNA.
